If you feel that your benefits eligibility or enrollment is incorrect, you have the right to file a written appeal. A new appeal form is now available for use when submitting a benefits appeal, located on your benefits portal in the Reference Center (located on the top bar) in the Forms folder.
The decision on your appeal will consider all details, documentation, and the IRS guidelines and regulations that govern the Republic Services benefit plans.
The appeal process applies to U.S. non-union employees and to employees who are covered by a collective bargaining agreement that provides coverage in the Company’s benefit plans. If you are covered by a collective bargaining agreement, please refer to your union contract, call the Benefits Service Center or visit your benefits portal, as benefits options, rates, and enrollment rules may differ.
Here’s what you need to know about the benefits appeal process:
- Your appeal should include a detailed description of events including any extenuating circumstances that caused a failure to enroll or loss of coverage/eligibility under the Plan. (Extenuating Circumstances are situations beyond your control, such as natural disasters (earthquake, flood, fire), acts of civil or military authority, war, or terrorism.
- Documentation must be attached supporting your appeal, including the relevant information and circumstances.
- Appeals must be submitted in writing within 45 days of the occurrence date.
- The Appeals Processing Team will review your appeal within 7 business days. You will receive a notification of the decision by mail or a message in your Message Center on your benefits portal.
Please note that this process and form are not for denied claims (such as a medical or dental claim). Denied claims should be submitted to the appropriate carrier, outlined by the Claims Filing and Appeals Procedures included in the Summary Plan Descriptions. Summary Plan Descriptions are available in the Reference Center on your benefits portal.
